- Yes, I want to be a donor - You want to be a donor.
- No, I do not want to be a donor - You do not want to be a donor.
- My partner or family will decide after my death - Your partner or family may make the choice upon your death.

Read more here about making your choice in the Donor Register.
How can I view or change my choice in the Donor Register?
You can always view and change your choice in the Donor Register yourself. You can do this with your DigiD on the Donor Register website.
Without a DigiD, you can request your choice via this application form on the Donor Register website. You will then receive a letter at home with your choice at the address where you are registered with the municipality. To change your choice without a DigiD, fill out a donor form. You can request this form online, pick it up at the town hall, or request it via the information number0900 821 21 66.
Which organs and tissues can I donate?
After your death, you can donate these organs: liver, heart, lungs, pancreas, kidneys, and small intestine. Donor organs help sick patients who would die without this organ.
You can donate these tissues after your death: eye tissue, heart valves, major blood vessels, skin, bone, cartilage, and tendon tissue. Tissues are used to improve the quality of life for patients.
You decide for yourself which organs and tissues you do or do not want to donate. You can register this in the Donor Register.
Can I be a donor if I have an illness?
Yes, you can be an organ and tissue donor if you have an illness. Your illness may affect which organs and tissues you can donate. A doctor will always examine what is possible.
Up to what age can I donate?
Age is, in principle, not that important for donation. It is about whether your organ or tissue is still in good condition. A doctor will always examine whether the organs and tissues are still healthy enough before the donation.
